Two slots turns a list into a pairing problem
A trinket on its own is a minor buff. A pair is a build, and the number of available builds grows far faster than the list does:

Forty-nine trinkets produce 1,176 distinct two-slot combinations. Nobody works through that by choosing, which is why most players settle on two or three loadouts and stop exploring somewhere around the fifth run.
Spin twice with removal on and the wheel deals a pair without repeating. Since the game saves three loadout presets, three double-spins fill all three slots and give you a week of builds you would not have assembled.
Where the wheel is unevenly weighted
Trinkets sort into six categories: skill check, movement speed, stamina, stealth, extraction, and other. Those categories are not evenly represented, so a random pair frequently doubles up on one axis and leaves another bare.
That is the interesting failure. A pair covering the same stat twice is usually worse than a pair covering two, and being handed a doubled-up build forces you to play around a weakness rather than avoid it. Extraction and movement dominate most lists; stealth is thin.
Weights fix it if you would rather build deliberately. Entering 3, Diary against 1 elsewhere tilts toward a category you are trying to learn, and the pair still arrives without you choosing both halves of it.
Locked in once the run starts
Trinkets cannot be swapped once a floor begins. Whatever the wheel gave you is what you have for the whole run, which is the difference between this and a randomiser you can quietly override.
That constraint is what makes a spun loadout a real challenge rather than a suggestion. A pair that turns out badly on floor three has to be played through, and playing through a bad build teaches you the floor rather than the loadout.
One entry is a pure handicap. The Brick is the only trinket in the game that applies a movement penalty with no benefit attached, so drawing it means running a slot down. For a deliberate challenge that is a fine result. For a serious run it is worth weighting out rather than deleting, so it stays possible.
Where trinkets come from
Two routes, and neither is quick. Most are bought from Dandy's Store with Ichor, several gated behind conditions like surviving a set number of floors. The rest require 100 percent research on a specific Twisted, which means surviving encounters with it repeatedly and collecting research along the way.
That grind is why a full-list wheel misleads a newer account. Spinning 49 trinkets when you own twelve returns something unusable most of the time. Trim the list to what you have actually unlocked and every remaining entry gets better odds: cut to twelve and each carries 8.3 percent.
One exception is worth knowing: a single trinket comes from joining the developer's Roblox group rather than through play, and it is the only one that can be freely removed and re-added.
Pairing trinkets with a toon
The loadout is half a build. Which toon carries it is the other half, and spinning both removes the pull toward pairing your comfort trinkets with your comfort toon.
